PRP- systematic review























The American Journal of Sports Medicine에서 PRP에 대한 systematic review article이 나왔네요!

제가 알기로는 PRP에 대한 시스테마틱리뷰는 최초라서 반가운 마음에 올려봅니당

2014년 12월 18일에 출판된 논문이네요.

내용을 다 보지는 않았지만, forest plot까지 있는 걸 보니 시스테마틱리뷰 뿐만 아니라 메타분석까지 했나보네요!

내시경을 이용한 경피적 요추 절제술에서의 학습곡선 Learning curve for percutaneous endoscopic lumbar discectomy

Brief review

이 논문은 PELD에서의 learning curve에 대한 논문 입니다.
요새 계속 PELD learning curve에 대한 논문을 보고 있는데, 각 논문별로 언급된 항목에 조금씩 차이가 있네요. 여기서는 수술시간, 1년 후의 임상적 결과, 수술 성공률, 수술 실패율, 합병증 발생률, 재발률 등의 항목을 조사하였습니다. 
이 논문이 2008년에 출판되었는데, 최근에 출판된 PELD learning curve관련 논문들과 큰 차이는 없어보입니다.

Clinical success를 아래의 기준으로 판단했습니다.
1. 통증지수(VAS)의 향상률이 2점 이상
2. 장애지수(ODI)의 향상률이 25% 이상
3. 환자의 주관적인 회복률이 50% 이상
4. 주요한 합병증이 없음
그런데 주관적인 회복률을 어떻게 평가했는지 안나와있네요.. clinical outcome 평가 시 처럼 환자에게 전화하여 환자가 느끼는 회복률을 적용한 것 일까요

그리고 특이한점은 일반적으로 의학통계에 널리 사용되는 SPSS가 아니라 SAS라는 프로그램을 사용하여 분석을 했습니다.

Background of data
Percutaneous endoscopic lumbar discectomy (PELD) is one of the surgical options for soft lumbar disk herniation, but the learning curve is perceived to be steep.


Methods















Subjects: 51 PELD cases performed for single-level intracanalicular lumbar disk herniation causing radiculopathy.
Divided into 3 groups: each 17 patients
Evaluating parameters: operating time, failure rate, complication rate, and 1-year reherniation rate.
1 year clinical success rate was assessed by telephone interviews






























Results
4 patients required subsequent open discectomy due to PELD failure.
2 minor complications.
1 year clinical success: achieved in 42 of the 47 patients
Reherniation developed in 5 patients
-A significant reduction in operating time was observed after 17 patients had been treated (p = 0.0004).
Failure rate, complication rate: no significant differences between the 3 groups
1year clinical success rate, reherniation rate: no significant differences



Conclusion

The PELD learning curve seems to be stable and acceptable with proper pre-PELD training.

Study Design
A single-centre, prospective, non-comparative study


Objective
To evaluate the performance and safety of the Memory Metal Minimal Access Cage (MAC) in Lumbar Interbody Fusion.


Summary of Background Data
Interbody fusion cages in general are designed to withstand high axial loads and in the meantime to allow ingrowth of new bone for bony fusion.
In many cages the contact area with the endplate is rather large leaving a relatively small contact area for the bone graft with the adjacent host bone.
MAC is constructed from the memory metal Nitinol and builds on the concept of sufficient axial support in combination with a large contact area of the graft facilitating bony ingrowth and ease in minimal access implantation due to its high deformability.


Methods
Subjects: 25 patients with back and radicular leg pain from a single level degenerative lumbar disc underwent an interbody fusion using MAC and pedicle screws.

Clinical evaluation: Oswestry Disability Index (ODI), Short Form 36 questionnaire (SF-36) and pain visual analogue scale (VAS) scores

-The interbody fusion status was assessed using conventional radiographs and CT scan.
-Safety of the device was studied by registration of intra- and post-operative adverse effects.






Results
Clinical performance: improved significantly (P < .0018)
-CT scan confirmed solid fusion in all 25 patients at two year follow-up.
-In 2 patients migration of the cage occurred, which was resolved uneventfully by placing a larger size at the subsequent revision.


Conclusions

We conclude that the Memory Metal Minimal Access Cage (MAC) resulted in 100% solid fusions in 2 years and proved to be safe, although two patients required revision surgery in order to achieve solid fusion.

Disc height and sagittal alignment in operated and non-operated levels in the lumbar spine at long-term follow-up: a case-control study

Brief review

이 논문은 요추유합술을 시행한 환자 3명과 시행하지 않은 환자 25명을 수술시행 9년 후 추적관찰하여 임상적, 방사선학적 결과를 측정한 연구 입니다.

방사선학적 결과는 Distortion Compensated Roentgen Analysis (DCRA)라고 하는 지표를 이용해서 측정했고 임상적 결과는 ODI, VAS(back)으로 측정하였습니다.

결과적으로 디스크의 높이는 유합술을 시행한 환자의 모든 분절에서 감소하였으며,
유합술을 시행한 환자와 시행하지 않은 환자간의 디스크의 높이나 시상면 정렬에는 차이가 없었습니다.

요추 디스크의 퇴행은 유합술을 시행 9년 후, 해당 환자의 모든 분절에서 증가하였으며, 임상결과 또한 좋지 않다는 결론 입니다.

이 논문에서는 방사선학적 변수(disc height, sagittal alignment )을 측정 시 DCRA라고 하는 지표를 사용한 점이 특징 입니다. DCRA에 대한 자세한 측정 방법은 아래의 그림 참고하세요.

요추 수술 분절과 수술 분절에서의 디스크의 높이와 시상면 정렬에 대한 장기 추적 결과: 사례통제 연구
*Authors: Froholdt A, Brox JI, Reikerås O, Leivseth G.


PURPOSE
To compare lumbar disc degeneration at 9-year follow-up in patients with chronic low back pain who had instrumented lumbar fusion or no fusion.


MATERIAL AND METHODOLOGY
1. The main outcomes: 1)disc height, 2)sagittal alignment
-measured by Distortion Compensated Roentgen Analysis (DCRA)

2. Secondary outcome: Oswestry Disability Index and VAS back pain.











RESULTS
Subjects: 48 patients (23 fusion + 25 no fusion)
Follow-up period: pre and 9 year
including level: L2- S1
-Disc height was reduced at all levels independent of fusion.
-No difference in disc height and sagittal alignment was observed between patients fused and not fused.
-There were weak correlations, ranging from 0.04 to 0.36, between clinical and radiological parameters.


CONCLUSION
Lumbar disc degeneration increased at all measured levels independent of fusion and correlated poorly with clinical outcome.
